Written Exam Registration Form

AATI SPECIAL WRITTEN CERTIFICATION EXAM SESSION REGISTRATION, POLICIES AND PROCEDURES

1. To register, complete the provided form. Register between 8:30 AM and 8:30 PM, Monday–Thursday. Payment: major credit card, debit card, company checks, or money order.

2. Exam date and time are indicated on the registration. Arrive at 6801 W. 20th Ave, Hialeah, FL 33014 at least 15 minutes before your scheduled time.

3. By registering you accept AATI's terms regarding exams and results, including the right to confirm certification status.

4. Up to two (2) certification exams or three (3) recertification exams per session. Space is limited — first come, first served.

5. Arrive on time. Once the exam session begins, no late entries will be permitted.

6. Please have a valid email address at time of registration.

7. You are responsible for knowing your exam date. Missed tests cannot be rescheduled. Call 305-362-5519 or email admission@aati.edu.

8. If you miss or fail, you must register again and pay the applicable fee.

9. Fees are non-refundable. No credit for missed exams.

10. Present a current government-issued photo ID. Name must match registration.

11. No electronic devices, manuals, notes, or personal items in the testing room.

12–14. No guests, food, drinks, weapons, or large bags in the testing area.

15. Official score report within 30 days. Certificate issued if you pass.

16–17. AATI reserves the right to cancel scores or revoke certification for misconduct or fraud. Exam material is confidential.
